- new places inspired by Robert E. Howard's writing, such as the Nameless City, Kamula, the ruins of Lemuria and Atlantis
- places from H.P. Lovecraft's Cthulhu mythos, such as R'lyeh and the City of the Elder Things
- a system to build your own ruins and cursed cities
- powerful entities such as the servants of Bit Yakin, Thugra Khotan and Gyatha, Master of the Black City
- rules for exploring ruins and avoiding traps
- famous treasures such as the Teeth of Gwahlur, the Dagger of Derketa and the Staff of Epemitreus, plus a treasure generation system
